Cerebelite Aguda ou Ataxia Cerebelosa Aguda Pós-Infeciosa? Abordagem de Casos Clínicos Pediátricos Paradigmáticos

A cerebelite aguda é uma doença rara, resultante da disfunção cerebelosa ou das suas conexões, de causa infeciosa ou inflamatória, com manifestações variadas e evidência de inflamação predominantemente cerebelosa na ressonância magnética (RM). Pertence ao espetro da ataxia cerebelosa aguda pós-infeciosa, uma doença autolimitada, cuja abordagem terapêutica é individualizada. Descrevem-se três casos clínicos de ataxia cerebelosa em idade pediátrica. No caso 1, existe um contexto pós-infecioso associado ao vírus Epstein-Barr. No caso 2, existe um contexto pós-infecioso possível por Mycoplasma pneumoniae, sendo a RM reveladora da existência de inflamação cerebelosa. Em ambos foi utilizada corticoterapia. O caso 3 apresenta-se com clínica cerebelosa grave pós-infeção pelo vírus varicela zoster, sendo tratada com imunoglobulina humana. A evolução foi favorável em todos. Estes casos demonstram o espetro clínico variável da ataxia cerebelosa pós-infeciosa, bem como as diferentes opções terapêuticas utilizadas, dependentes da apresentação clínica e das características individuais das crianças afetadas.Downloads
